I think it's possible that if you miss the boss by quite a bit, that the pin will appear to go in, but it's just pushed up against the side of the boss.

I know I am very careful to see where the end of the pin is going. (i.e. I can't see it behind the wheel)

That's exactly what I think went wrong Andy, assumed that as long as the pin went "in" that it was in place. How wrong I was! Recently bought a 2007 RRS HSE TDV6 for a Pimp-My-Ride-Project.
DONE: Meduza front bumper, AB rear bumper, AB tailpipes, calipers repainted, rear upper tailgate upgrade, side steps, AB front grille, AB side vents, rear LED lights, clear side repeaters, integrated daytime running lights, LED headlights.
TO DO: color code sills and pillars.

yeah I fiddled a lot trying to get it "in" and suspect It may have been against the lug which was sufficient to undo the old crank bolt. however, when I came to go the opposite direction to tighten the new crank bolt, it did not lock. I rolled it back and took out the tool. when I put it back I rocked the crank slightly and pushed the tool In so it was flush on the outside and I ran a bolt through the starter hole to hold it. the crank wouldn't go either direction and I was able to tighten the bolt with a 3/4 bar and a steam pipe 4 ft long Very Happy
